Frustrating Symptoms (KOEO KOER Test)

Car starts up and runs very rich (11.5-13.0:1) during warm up (no-drivability). Once warmed up idles normally, and no issues. If driven normally for about 10-15 minutes (stop and go) the car will start idling up and down slightly; switching between 12.5-13.5:1 and 16.5-18:1.

If given full throttle it loads up and blows out spark (or at least feels like spark blow out), builds boost slightly and backfires loudly.

KOEO Codes Reveal (At Operating Temp)
31 - EGR Valve Position/Pressure Feedback EGR Circuit Below Minimum Voltage. 0.24 volts
67 - Park Neutral Position Switch Circuit Open; A/C On -Manual
81 - Secondary Air Diverter Circuit Failure
82 - Secondary Air Bypass Circuit Failure
85 - CANP Circuit Failure

KOER Codes (Op. Temp)
21 - Engine Coolant Temperature (ECT) Sensor Out Of Self Test Range. 0.3 to 3.7 volts (Coolant Temp Sensor is Brand New, and why didn't this show in the KOEO?)
31 - EGR Valve Position/Pressure Feedback EGR Circuit Below Minimum Voltage. 0.24 volts
42 - System Indicates Rich -passenger side(R), No o2 Sensor Switching Detected. always rich -passenger side (O2's are brand new)
92 - o2 Sensor Circuit Indicates Rich -driver side(R), No o2 Sensor Switching Detected. always rich -driver side (Once again O2's are brand new)

Problems only started occurring after I replaced the head gaskets, car ran horrible for the first time I started it and drove it, found out I forgot to hook up the O2 ground. Did that and then it started doing this.
