Has the intake manifold been removed recently. I had the same symptoms and it turned out to be an intake manifold leak.

New fuel filter, and yes new intake, but it was doing this before the intake swap. I'll check on the pcv valve

Manifold gasket is good and new PCV valve...Still doing it. Anymore suggestions?

cleaned the iac valve and maf meter? if so, check the fuel pressure when you very first start up the car

Yes, both are clean and fuel pressure stays a constant 38-40 psi at idle
